For decades, Outreach Community Health Centers (OCHC) has served as a pillar of health and wellness in the Greater Milwaukee community. Committed to providing comprehensive primary, family, and behavioral health services, OCHC ensures that individuals and families—regardless of insurance status—receive the care they need. With a steadfast mission to address social determinants of health, OCHC offers a holistic approach to medical care, behavioral support, and community outreach.
A Commitment to Accessible Healthcare
OCHC primarily serves patients residing on Milwaukee’s Northwest side, focusing on 11 key zip codes that reflect significant socioeconomic challenges. Many of the individuals served are publicly insured or uninsured, emphasizing the need for accessible, affordable healthcare solutions. Rooted in its origins as a Healthcare for the Homeless site since 1979, OCHC continues to support the city’s homeless population, which accounts for 14% of its total patients.
Through its Mobile Health Unit (MHU), OCHC extends its reach beyond clinic walls, providing primary care services to homeless shelters, treatment centers, and youth-focused organizations. This initiative underscores OCHC’s commitment to meeting vulnerable populations where they are, ensuring no one is left without medical attention.
School-Based & Satellite Services
Recognizing the growing need for adolescent mental health support, OCHC offers school-based health services, including behavioral health and primary care. In 2023, partnerships with Carmen Schools of Science & Technology and the Hmong American Peace Academy enabled students to access quality healthcare directly within their school environments. These efforts are crucial in addressing mental health concerns among Milwaukee’s youth and improving overall access to essential medical services.
Featured Programs: Enhancing Patient Care
OCHC offers a variety of specialized programs designed to support diverse healthcare needs:
Prenatal & Child Care Coordination
Designed for expectant mothers and young children up to age seven, this program provides essential healthcare services for families facing housing instability or high levels of stress. By ensuring access to prenatal and pediatric care, OCHC helps improve birth outcomes and long-term child wellness.
Hypertension Management
Through increased education, care management, and the distribution of Bluetooth-enabled blood pressure monitors, OCHC empowers patients to monitor and manage their blood pressure effectively, reducing long-term health risks.
Diabetes Support & Education
OCHC’s Certified Diabetic Education program offers personalized guidance for individuals managing diabetes. Led by experienced educators and dietitians, these sessions cover nutrition, blood sugar management, and diabetes prevention. Additionally, the Diabetic Eye Exam Program provides essential screenings to detect early signs of diabetes-related vision impairment, helping prevent blindness through early intervention.
Recognized Excellence: Patient-Centered Medical Home (PCMH)
In August 2023, OCHC achieved PCMH recognition from the National Committee for Quality Assurance. This distinction highlights the organization’s commitment to coordinated, patient-centered care, ensuring individuals receive comprehensive and continuous healthcare tailored to their specific needs. PCMHs are proven to enhance chronic disease management, reduce healthcare costs, and improve patient satisfaction.
Advancing Research: All of Us Research Program
As part of a national effort to ensure diversity in health research, OCHC partners with the All of Us Research Program at the Medical College of Wisconsin. This initiative seeks to improve precision medicine by gathering comprehensive health data from diverse communities. Patients interested in participating can learn more by visiting JoinAllofUs.org or contacting OCHC for enrollment details.
A Vision for a Healthier Milwaukee
OCHC’s unwavering dedication to healthcare equity and community wellness makes it an invaluable resource for Milwaukee residents. Through its integrated services, patient-centered approach, and commitment to innovation, OCHC continues to break barriers in healthcare accessibility, ensuring that all individuals—regardless of background or financial situation—receive the compassionate, high-quality care they deserve.
